Any health, social care or housing worker, or worker for a charity, who is supporting the individual requiring assistance, is capable of assessing their needs and willing to act on their client’s behalf can make a Grant Application on their behalf.


Step by Step Instructions for Referrer


We do not accept applications directly from individual members of the public. All applications must be submitted on behalf of the individual by professional supporters-Referrers. The following supporters are eligible to be Referrers.

  • Staff employed by statutory health, social care or advice services-a Specialist Nurse, Occupational Therapist, Physiotherapist or Social Worker. A doctor can be a referrer but only if he/she is able to submit and process the application.
  • Professional workers from charities that provide a health, social care or advice service-for example Age UK, Stroke Association, Royal British Legion, Muscular Dystrophy Campaign
  • Citizens Advice workers
  • Workers from Tenancy Support, Care and Repair organisations and Housing Associations

  • Written confirmation of medical diagnosis will be required from all non-medical agencies. More information can be found in our FAQs .



    There is a two stage application process-the Referrer Form and the Grant Application Form


  • Name and address of your client-the person needing assistance (Applicant)
  • Referrer contact details
  • Client’s medical diagnosis (no more than 30 words)
  • Client’s circumstances (no more than 150 words)
  • Information on what your client needs and why you are seeking a grant (no more than 100 words)
  • Payee details-the grant can be paid either to a supplier/contractor or to the referrer’s agency. It cannot be paid direct to the beneficiary. We prefer to pay grants by BACS.
  • Attachments, for example Occupational Therapy Report, Quotation from Supplier or Contractor in soft copy form. You will need to provide separate written confirmation of the client’s medical diagnosis if you work for a non-medical agency

  • Client’s name , address, contact telephone number, date of birth and status
  • Purpose for which grant is sought
  • Total cost of the equipment or work-attach Quotation from Supplier if this has not already been provided with the Referrer Form
  • Household details-who lives with the client and their relationship
  • Type of accommodation
  • Total household weekly income
  • Sources of income
  • Savings
  • Debts
  • Other charities approached for funding
  • Whether there is any statutory financial assistance for this item

  • Please ensure that you have any attachments e.g. Occupational Therapy Report, Quotation, ready in a soft copy version, including scanning any relevant documents, before you begin to complete the Form/s


    How to start the application process

    As the Referrer, when you have submitted the completed Referrer Form you will receive an email with a link to the Grant Application Form. This form will need to be completed by you, the Referrer, and the Applicant (the person requiring assistance).

    When both parts of the Grant Application (the Referrer Form and the Grant Application Form) are submitted to Independence at Home the application will then be assessed by Independence at Home staff.
