Grant Application Form
Independence at Home welcomes grant applications on behalf of adults and children living with disabilities and long-term illness anywhere in the UK.
We do not accept applications from individual beneficiaries, their family members, friends, or colleagues.
If you are a health or social care professional or an advice worker from a registered charity/agency, please complete and submit the following form on their behalf.
You will need to be able to upload:
- evidence of medical condition;
- a quotation on company letterhead, specifying the equipment or works that your client (the Applicant) needs;
- a copy of your organisation’s current Safeguarding Policy.
The option to save a partly completed form and come back to it is available. You will be asked to create a new password each time you revisit the form. This ensures that you return to the latest version of your application.
When you have finished completing the form, we strongly recommend that you print a copy for your own records and save a copy to a file on your computer.
Please also check your junk or spam email folders for correspondence related to your application. It is important that all communications are received in a timely manner to facilitate the processing of your application.
