Grant Application Form

Independence at Home welcomes grant applications on behalf of adults and children living with disabilities and long-term illness anywhere in the UK.

We do not accept applications from individual beneficiaries, their family members, friends, or colleagues.

If you are a health or social care professional or an advice worker from a registered charity/agency, please complete and submit the following form on their behalf.

You will need to be able to upload:

• evidence of medical condition;
• a quotation on company letterhead, specifying the equipment or works that your client (the Applicant) needs.

You can save a partly completed form at any point and come back to it when you are ready. When you have finished, please print a copy for your own records.

Kindly ensure you review your junk or spam email folders for correspondence related to your application. It is imperative that all communications are received in a timely manner to facilitate the processing of your application.
